Predictive cytogenetic biomarkers for colorectal neoplasia in medium risk patients

چکیده
RATIONALE DNA damage and chromosomal alterations in peripheral lymphocytes parallels DNA mutations in tumor tissues. OBJECTIVE The aim of our study was to predict the presence of neoplastic colorectal lesions by specific biomarkers in "medium risk" individuals (age 50 to 75, with no personal or family of any colorectal neoplasia). METHODS AND RESULTS We designed a prospective cohort observational study including patients undergoing diagnostic or opportunistic screening colonoscopy. Specific biomarkers were analyzed for each patient in peripheral lymphocytes - presence of micronuclei (MN), nucleoplasmic bridges (NPB) and the Nuclear Division Index (NDI) by the cytokinesis-blocked micronucleus assay (CBMN). Of 98 patients included, 57 were "medium risk" individuals. MN frequency and NPB presence were not significantly different in patients with neoplastic lesions compared to controls. In "medium risk" individuals, mean NDI was significantly lower for patients with any neoplastic lesions (adenomas and adenocarcinomas, AUROC 0.668, p 00.5), for patients with advanced neoplasia (advanced adenoma and adenocarcinoma, AUROC 0.636 p 0.029) as well as for patients with adenocarcinoma (AUROC 0.650, p 0.048), for each comparison with the rest of the population. For a cut-off of 1.8, in "medium risk" individuals, an NDI inferior to that value may predict any neoplastic lesion with a sensitivity of 97.7%, an advanced neoplastic lesion with a sensitivity of 97% and adenocarcinoma with a sensitivity of 94.4%. DISCUSSION NDI score may have a role as a colorectal cancer-screening test in "medium risk" individuals. ABBREVIATIONS DNA = deoxyribonucleic acid; CRC = colorectal cancer; EU = European Union; WHO = World Health Organization; FOBT = fecal occult blood test; CBMN = cytokinesis-blocked micronucleus assay; MN = micronuclei; NPB = nucleoplasmic bridges; NDI = Nuclear Division Index; FAP = familial adenomatous polyposis; HNPCC = hereditary non-polypoid colorectal cancer; IBD = inflammatory bowel diseases; ROC = receiver operating characteristics; AUROC = area under the receiver operating characteristics curve.

Folate metabolism polymorphisms influence risk of colorectal adenoma recurrence.
Folate intake is inversely related to risk of developing colorectal neoplasia. Associations between risk of colorectal neoplasia and polymorphisms in genes coding for enzymes involved in folate metabolism have also been reported, suggesting a relationship between genotype and development of colorectal neoplasia. To further investigate the effects of folate metabolism genotypes on colorectal neo...
متن کاملRole of Molecular Biomarkers in Colorectal Cancer
Colorectal cancer (CRC) is amongst the most widespread cancers and is a most common cause of cancer associated mortality universally. Since previous decades, it has been cleared that CRC develops owing to the buildup of a series of genetic and epigenetic changes in the normal colonic epithelium. Regardless of the current development in surgery and therapies, overall survival of end stage CRC pa...

Colorectal cancer (CRC) is among deadliest cancers all over the world. Regarding its high mortality, some researches have focused to discover new applicable diagnosis methods. In this regard, circulatory miRNAs has been received huge consideration as promising biomarkers for early detection of CRC. The study aimed to evaluate the expression level of miRNA-7, miRNA-93, and miRNA-409 in plasma of...

Background: In chronic myeloid leukemia (CML), the influence of BCR-ABL1 transcript type, Sokal risk score and smoke on disease phynotype and cytogenetic response to treatment is still unknown and arguable. The objective of this study was to determine the prognostic significance of transcript types, risk score and smoking status among patients with CML treated with different tyrosine kinase inh...
متن کاملCardiovascular risk markers and mechanisms in targeting the COX pathway for colorectal cancer prevention.
COX-2 inhibition reduces the incidence of colorectal neoplasia. The increased risk of thrombotic cardiovascular events produced by selective or nonselective COX-2 inhibitors, however, has confounded the consideration of employing them in cancer prevention. Developing a strategy for preventing colorectal cancer by inhibiting COX-2 depends on research advances in several key areas, including pred...
